This clinic, formally known as Integrated Health Care - West, will work together with you so it is as easy as possible for you to receive health and social services in your neighborhood, close to home. We provide holistic, integrated, compassionate physical and mental health services. We are your healthcare partners, helping you to manage and understand your health problems, and guiding you through the healthcare system. As an IHC site, what sets us apart from other Mile Square Health Center (MSHC) clinics is that we are fully staffed by outstanding Nurse Practitioners. Nurse Practitioners are registered nurses with advanced degrees, certification, and clinical training to provide health care services to individuals of all ages. Nurse Practitioners promote a comprehensive approach to health care and emphasize the overall health and wellness of their patients.

Community Health Centers are neighborhood-based health care clinics that provide comprehensive primary and preventive care to underserved populations, such as the uninsured, low income, Medicaid recipients, and racial and ethnic minorities. In addition to primary and preventive care, most community health centers provide prenatal care, pediatrics, and chronic disease management. Some community health centers serve special populations, including the homeless, gay and lesbian populations, people living with HIV/AIDS, and disabled individuals. The Statewide Coordinator for Deaf and Hard of Hearing Services provides training programs and additional materials in the area of mental health and deafness/hearing loss. The program serves individuals who are deaf or hard of hearing and who also have mental health needs. Department of Human Services (DHS) - Division of Developmental Disabilities
The Division of Developmental Disabilities Hotline 1-888-DD-PLANS (1-888-337-5267) (TTY 1-866-376-8446) is available to persons and the family of people with developmental disabilities to answer questions on community programs, respite care, and other state services available to persons with a disability. UIC Division of Specialized Care for Children (DSCC)
UIC Division of Specialized Care for Children (DSCC) has 13 regional offices across the state where care coordinators are available to assist families with medically eligible children (birth to age 21 years) access needed specialty services. The Core Program is the major focus of DSCC and offers care coordination and cost-supported diagnosis and treatment for children with chronic health impairments determined eligible for program support. The Home Care Program offers coordination and support for in-home medical care of technology-dependent children who would otherwise have to remain in a hospital or skilled nursing facility. The Children's Habilitation Clinic, located on the west campus of the University of Illinois at Chicago, within the Children and Adolescent Section of the Out-Patient Center, provides comprehensive diagnostic services to children with complex disabling conditions and provides ongoing rehabilitation and developmental management to those children to age 21. InfantSEE®, a public health program, managed by Optometry Cares® - the AOA Foundation, is designed to ensure that eye and vision care becomes an essential part of infant wellness care to improve a child's quality of life. Under this program, participating optometrists provide a comprehensive infant eye assessment between 6 and 12 months of age as a no-cost public service.
